When Artificial Tears Aren’t Enough: Advanced Treatments for Dry Eye

Most of the time, dry eye is not only caused by not having enough tears. Inflammation and the quality of your tears also play a big role. Over 80% of dry eye cases are linked to problems with the meibomian glands, which are small oil glands along your eyelids. These glands produce an oil that sits on top of your tears, helping them dry more slowly.

If these glands become clogged or inflamed, the oil layer does not work well. Your tears dry up quickly, even with a lot of eye drops. This is why it is important to understand what is happening on the surface of your eye before choosing advanced treatments.

Prescription Drops That Fight Inflammation

These drops do more than just wet the eye; they target the inflammation that keeps you uncomfortable.

Here are a few common options:

  • Cyclosporine. It calms the immune response on your eye’s surface and helps your body produce more of its own tears over time.
  • Lifitegrast. It blocks a specific inflammatory signal that can worsen dry eye.
  • Miebo. A newer drop that coats the eye and slows tear evaporation. It is especially helpful if your meibomian glands are clogged.

These drops take time. Full results may need weeks or even a few months. But many people find the relief is real and long-lasting.

Blocking the Drain

Your tears do not just disappear from the surface of your eye. They drain away through tiny openings called puncta, located in the corners of your eyelids. Sometimes, the problem is not that you make too few tears. It is that the tears drain away too fast.

Punctal plugs are small devices, about the size of a grain of rice, that fit into these drainage openings to slow down tear loss. Putting them in only takes a few minutes at the doctor’s office. The procedure is painless and can be reversed. By keeping your natural tears and eye drops on your eyes longer, these plugs can provide real relief.

Treatments That Target the Glands

If the oil glands along your eyelids are clogged, prescription drops alone may not clear them. Your doctor might suggest an in-office treatment instead.

  • Intense pulsed light (IPL). Gentle pulses of light warm the thickened oils and reduce inflammation around the eyes. IPL has been used in skin care for years and is now FDA-approved for the treatment of dry eye.
  • LipiFlow. This device warms the inner eyelids and simultaneously massages the outer eyelids. It helps push out clogged oils. A few sessions, spaced several weeks apart, are common.

Specialty Lenses for Severe Cases

For some people, the surface of the eye is so irritated that even blinking hurts. Scleral lenses can help in these cases. These large, custom-made contact lenses cover the whole cornea and hold a layer of fluid against the eye. This keeps the eye moist all day. While scleral lenses do not cure dry eye, they can bring major relief when other treatments have not worked.

Serum Tears

If the surface of your eye is badly damaged, sometimes the best treatment comes from your own body. Autologous serum tears are made from a small amount of your blood. The blood is processed to obtain the serum, which contains growth factors, vitamins, and proteins like those in your natural tears. These drops help your eye heal, not just stay moist.
